There are three types of intake choices at Both Hands Healing Arts:

A.  CASH – Not running this through insurance and paying at time of service with cash or check.
B.  HEALTH INSURANCE – Running this through your health insurance and paying your co-pay at time of service with cash or check.
C.  PIP – Running this through an accident insurance claim.

Please fill out and bring ALL of the intake information needed for your type of billing category.

A.  CASH – If you are not running this through insurance and paying at time of service with cash or check, you need to fill out and bring:

1.  Health History Intake Form

B.  HEALTH INSURANCE -If you are running this through your health insurance and paying your co-pay at time of service with cash or check, you need to fill out and bring:

1.  Health History Intake Form

2.  Insurance Intake Form

3.  Your insurance card

4.  A prescription from your doctor, chiropractor or naturopath.

**Failure to fully fill out forms and bring in all paperwork might result in being denied by insurance company and being responsible to cover your massage expenses yourself.**

C.  PIP – If you are running this through an accident insurance claim, you need to fill out and bring:

1.  Health History Intake Form

2.  Insurance Intake Form

3.  A prescription from your doctor.
